SasaCare have been providing services for Ndis Participants, process of becoming an NDIS registered and provide a range of services to NDIS clients to support them to reach their goals and build their capacity. Sasacare was created to support people on the NDIS to support choice and control and ensure their independence is always upheld. Ensuring that all people with a disability have their needs met, goals reached and providing a quality service to all persons.

About the role

The purpose of the role is to provide Coordination of Supports and Recovery Coach services for people with NDIS.

You will have to find the referrals for the supports and services required by participants, their families, and their carers on an individual basis.

Key responsibilities will include:

  • Coordinate for the referrals for NDIS participants including mainstream, informal and formal support.
  • Coach participants through individual recovery plan development and implementation, assisting to build skills for self-direction and take control of their own lives.
  • Provide and facilitate referral pathways is the main goal.
  • Support implementation of services that best suit the participant’s needs and wants.
  • Engage with NDIA regional contacts, including Local Area Coordinators.
  • Maintain positive working relationships and linkages with community agencies, clinical services, and all other relevant providers to ensure effective communication, referrals and support coordination can occur.
  • Monitor and report progress against NDIS Goal Plans, budgets, and data collection.
  • Maintaining the direct interactions with Nsw Public Health, Social workers, Support coordinators.
  • Meet targets as determined for effective service provision and adhere to KPI’s.
  • Provide and maintain quality service and always portray a professional image on behalf of SasaCare.
  • Understand the “NDIS key terms” and provide different supports to your participants pending on their needs and requirements such as Chance of Circumstances and Internal Reviews.

Part-time hours: 20 per week

Salary: $53,026.00 – $106,406.00 per year
